​All payments will be due at the time of services. 

FORMS 
Intake forms are online and will require an email. Paperwork will then needs to be completed prior to your first appointment. If you do not have access to an email, internet, or computer then you will need to let the counselor know prior to your appointment that you will need to fill-out the paper forms in office 20 minutes prior to your scheduled appointment.

CANCELLATIONS
Failure to make your scheduled appointment or cancellations within 24 hours prior to your scheduled appointment will result in a late/cancel fee.
